Information, Assessment & Planning (IAP)

This service enables people with disability, who want to enjoy community life and expand their opportunities, to explore their goals and needs and how they can be met. Planners work with individuals, to identify their ambitions and gain a full understanding of their issues and what might need to be done to resolve them. From this shared understanding a plan is developed. The plan outlines the desired lifestyle and lays out the steps that will need to be taken to achieve it.

Not everyone wants or needs a plan. Some people just want information about what is available to them. The IAP team has a wealth of information about services that are available in local areas and can offer information and referral support.
