What is Information, Planning & Case Management?
Yooralla's Information, Planning & Case Management (IP&C) teams provide support to individuals who are seeking assistance with planning. This service enables people with a disability, who want to enjoy community life and expand their opportunities, to explore their goals and needs and how they can be met. Planners work with individuals, learning about their ambitions and gaining a full understanding of their issues and what might need to be done to resolve them. From this shared understanding a plan is developed. The plan outlines the desired lifestyle and lays out the steps that will need to be taken to achieve it.
Not everyone wants or needs a plan. Some people just want information about what is available to them. The IP&C team has a wealth of information about services that are available in local areas and can offer information and referral support.
In order to use Disability Services, a person must be assessed as eligible. If your eligibility has not yet been assessed, the IAP team may be able to assist. Assessment is kept as simple as possible and may just require a letter from a Doctor. Other assessments can be more complex and the team will work with the person to obtain the necessary information.
Referral and application
A person can call the service directly or request a service application to be sent from the IP&C service most local to them. The IP&C team will review the service application. Once the team reviews the application an IP&C team member will be appointed and it will be this person’s role to contact you to determine the best time for our service to be introduced.
