What is Specialised Case Management and Consultancy Service?
Yooralla's Specialist Case Management is a management service that works in partnership with the client, and where appropriate, the family to provide holistic intervention to explore life-time goals and ensure the right types of support are in place. The Case Manager wants to understand what is important to the client. They will discuss what is most appropriate to the clients abilities, needs and goals, and what supports will be required to resume and maintain an active role in the community (immediately and in the long-term). The Case Manager will assist the client to develop an individual plan that identifies how various services and supports work together towards achieving the client's goals. The Case Manager also works with the funding body Transport Accident Commission (TAC) or Department of Human Services (DHS) to put the client's future plan in action and review progress. Our service approach is based on person-centred practice.
Eligibility
Specialised Case Management and Consultancy Services are targeted to TAC Clients who have complex needs arising from significant Acquired Brain Injury and / or SCI or other disabilities and are entitled to some form of support following a transport accident. We will also provide Case Management to clients identified through DHS.
Referral and application
The TAC / DHS client may choose their Case Management Agency. TAC or DHS will contact us to inform of client requesting our service. We will negotiate with TAC/DHS the best time for Case Management to be introduced. TAC / DHS will then contact you to acknowledge that your request has been received, the name of your case manager and the date they will contact you to organise a meeting to discuss/assess your needs and goals, develop actions to meet these goals and identify options. This will involve ongoing liaison with the TAC Support Coordinator.
Fees
TAC / DHS will fund your Case Management Service.
