Support for Older Carers

What is Support for Older Carers?

The Support for Older Carer Program (SOC) is designed to assist carers, over the age of 60, to access generic and disability specific services for people they care for.

There are many people within the community who have not previously accessed services and may not be aware of what is available.

The aim of the SOC program is to assist carers in navigating the service system and assist in planning for the future.

Facilitators can assist with:

  • Working together to determine the needs and interests of your family.
  • Discuss options including existing informal and community connections, as well as generic supports and disability specific services.
  • Working together to develop a plan that will address both the needs of the carer and the person with a disability.
  • The SOC facilitator will provide continued support to your family, helping to review the plan as your needs change and evolve.

Eligibility

To be eligible for the program you need to meet the following criteria:

  • Carer must be the carer of a person with a disability, as defined by the Disability Act 2006.
  • Carer must be 60 years or older. The person with the disability must be 64 years of age or younger.
  • Australian Indigenous Carers must be 45 years or older.
  • Carer must be living within the Eastern Metropolitan Region of Melbourne.

Fees
